SAN DIEGO (AP) - Health care regulators say they have fined four
San Diego-area hospitals $25,000 each for preventable mistakes that
led to the death of one patient.
In one incident, a staffer failed to turn on a ventilator, causing a heart patient to die. In another, a patient fell off a tilted operating table during surgery.
The incidents occurred at Sharp Grossmont Hospital in La Mesa,
Scripps Green Hospital in La Jolla, Pomerado Hospital in Poway and
